After scheduling your appointment, you will receive an email with new client paperwork. For your convenience, you can fill out and sign the paperwork electronically and submit it before your appointment!

CANCELLATION & NO SHOW POLICIES:

We require 24-hours notice for cancellations so we can offer your appointment time to someone waiting to get help. If you cancel with less than 24 hours notice there is a $75 cancellation fee. If you fail to show up for your scheduled appointment, you will be charged $175. Of course certain situations will be considered, but please be considerate of our time and other clients wanting to get in for help!
